Alzheimer's disease, Dementia with Lewy Bodies, Vascular Cognitive Disorders (e.g. stroke), Frontotemporal Dementia, Parkinson's Disease Dementia, Delirium, Mild Cognitive Impairment (MCI), and Depressive Disorders are age-related neurological conditions that are associated with Neurocognitive Impairment and tend to progress over time. In geriatric populations, accurate and ongoing assessment of patients/family who experience progressive neurocognitive illness is essential for proper diagnosis, treatment, and follow-up care. This app is part of the cTrack Monitoring Series, created by experts at Metrowest Neuropsychology and designed for use by family, caregivers, and healthcare professionals. The app provides patient support through regular and careful monitoring of four important domains of functioning: mental ability, communication, emotional-behavioral, and daily activity. The program allows for storage of historical data, providing a metric for change over time, including changes associated therapeutic intervention or medication adjustment, changes in environment (e.g. housing), and changes due to the natural course of illness. The cTrack program generates easy-to-understand data for family and healthcare providers to enable and optimize patient care. The program also includes hundreds of helpful tips and recommendations that can be used by patients and caregivers to improve overall functioning and quality of life.
